Currently Enrolled in the HSA

If you’re currently enrolled in the HSA with HSA Bank and wish to transfer your UMR balance, simply complete the Direct Transfer Request Form and return it to UMR to request your funds be transferred to HSA Bank.

Please allow 4-6 weeks from the date you return the form for the funds to transfer into HSA Bank.

Not Currently Enrolled in the HSA

If you’re not currently enrolled in an HSA, but would like to open an account with HSA Bank to have your HSA funds with UMR transferred, you can transfer by following the steps below:

  1. Complete your HSA Bank Application.

  2. Complete the Direct Transfer Request Form and return it to UMR to request your funds be transferred to HSA Bank.

Note that you will need your HSA Bank account number for the transfer form. You will receive in the mail an HSA Bank Welcome Kit containing your HSA account number. If you don’t want to wait for the Welcome Kit to arrive, you can log in to the HSA Bank Member Website to obtain your HSA Bank account number two business days after completing your online application.
