You can request up to two debit cards for free during account opening with HSA Bank. One card is issued in your name; the other can be issued in your Authorized Signer's name. If you want a card, indicate this on the HSA Application and Eligibility Form under the "HSA Account Options" section. To obtain a card for your Authorized Signer, check the "Second Debit Card Option" in the Authorized Signer section of the form. If cards are not ordered during the application process, you will be charged a $6.00 card issuance fee.
Your debit card renews automatically every four years. The card renewal fee is $6.00 per card. HSA Bank may at its discreation decide not to reissue a card. If your card is not automatically reissued, it is typically due to incorrect address information, the previous card was inactive, or there are concerns regarding fraudulent or misuse of the card. If your card was lost or stolen, replacement fee is $12.00 per card.
To cancel one or both of your debit cards prior to renewal, please contact our Client Assistance Center during business hours at 1-800-357-6246 or via our Contact Us page at least one full month prior to the renewal date. To determine the renewal date, please look at the "valid thru" date on the face of the card for the month and year (ex: 10/09). In this example, the card renews in October 2009, so you would need to contact us on or before August 31, 2009 to cancel the card renewal.
If you already have an HSA with us, and need to order or replace a debit card, you may log in to Internet Banking to complete the Debit Card and Check Order Form or contact HSA Bank during business hours at 1-800-357-6246 to order your new cards(s). The cost of ordering a card after opening your HSA is $6.00.
