Order Debit Cards & Checks
Debit Card Order Information
Receive up to two free debit cards when you open your HSA. One can be issued to you and the other to your authorized signer. This information can be indicated on the HSA Application & Eligibility Form under “HSA Account Options.”
Order Additional Debit Card(s)
Log in to Internet Banking and complete the Debit Card and Check Order Form located under “Resources” in the top navigation bar. The cost for debit cards after account opening is $6.00 per card.
Renew Your Debit Card
Debit cards automatically renew every four years. If your card is not reissued, it is likely due to incorrect address information, inactivation of the previous card, or concerns regarding fraudulent or misuse of the card.
Cancel Your Debit Card
If your card was lost or stolen, or if you would like to terminate debit card access, a $12.00 fee will be applied to your account. To cancel your card or report it lost or stolen, contact the Client Assistance Center at (800) 357-6246.
Check Order Information
Log in to Internet Banking and complete the Debit Card and Check Order Form located under “Tools” in the top navigation bar. There you can purchase a book of 50 duplicate checks and 10 deposit slips for $7.95.
